Sports Medicine

The Lake Erie College department of athletics is pleased to introduce you to the online source for Storm sports medicine information. Below is a series of forms needed to be completed by every Lake Erie College student-athlete prior to participation in a sport during the 2007-08 academic year.

Before filling out the seven forms, please check out the policies and procedure letter to familiarize yourself with the requirements necessary on behalf of the student-athlete and/or parent/guardian.

In addition to the above forms, every student-athlete must return a copy of his/her insurance card (both front and back) prior to attending one of the on-campus physical dates.
