Sports Medicine Physical Dates
Every Lake Erie College student-athlete is required to attend the on-campus physical for his/her specific sport. If competing in more than one sport, the student-athlete must attend the earliest sport's physical.
The seven forms, located on the sports medicine home page, need to be printed, filled out and brought with the student-athlete to the physical. Additionally, a copy of the student-athlete's insurance card - both front and back - must be brought to the physical.
No student-athlete will be permitted to partcipate in ANY on-campus team activity, practice included, until all forms have been completed and returned, a copy of his/her insurance card is on file and a physical has been attended.
