Checklist for Successful Accommodations

The following Checklist for Successful Accommodations may be helpful in assuring that your accommodations are relevant and current.

• I have met with the Director of The Student Success Center and disclosed that I am a student with a disability.
• I have submitted current documentation to the Director of The Student Success Center I have worked with the Director of The Student Success Center and or staff in the development of current accommodations.
• I have received notification of the action by the College Disability Committee regarding my requested accommodations.
• I have met with my professors to discuss the approved accommodations.
• I have returned the signed Accommodation Plan/s to the Office of Academic Services.
• I have met with the Director of The Student Success Center and / or the Learning Center Assistant regarding revisions to my accommodations.
• I have scheduled an end of the semester conference with the Director of The Student Success Center, as needed.
