The Registrar’s office is working on providing fillable PDF versions of its most commonly used forms, which will be available soon.
Options for completing a fillable form:
If you have Adobe, download and save the form and use the digital signature function. Save with a new name and return the form to email@example.com.
If you do not, you can download Adobe Reader for free and create your digital signature. https://acrobat.adobe.com/us/en/acrobat/pdf-reader.html
Once you have it, download the form and save. Click in the signature field, choose "Sign with a Digital Id", configure new digital id, continue. Mac will ask you to create Apply KeyChain. Sign the document, save with a new name and email it back to us at firstname.lastname@example.org
